Preparation & service details

Have a prepared affidavit, insurance statement or claims document that needs notarization? We can meet you at a home, office or another agreed Greater Boston location to perform the requested notarial act.

Documents to discuss

  • Personal, witness and business affidavits
  • Prepared statements for insurance claims or proof of loss
  • Medical or records-custodian sworn statements
  • Prepared releases and settlement documents requiring notarization
  • Corporate declarations and affidavits

What happens at the appointment?

The notary reviews identification and the requirements of the requested act. A sworn affidavit generally involves an oath or affirmation and signing in the notary’s presence. Other documents may require an acknowledgment. Ask before signing in advance, and have the preparing attorney or recipient supply the required document and certificate wording.

What notarization covers

Notarization does not decide whether a claim is valid, make an agreement legally enforceable or guarantee court acceptance. The signer is responsible for the statement, and the receiving organization determines its document requirements. Questions about legal effect or settlement terms belong with your attorney.

Prepare your request

Tell us the document type, signer count, city or town, preferred appointment time and deadline. Include any witness, printing, scanback or return-delivery instructions. We confirm the scope, travel fees and available appointment before you book.

For insurers, attorneys and business teams

Identify the person making the statement and provide the prepared affidavit, proof-of-loss form or claims document. Include the requested notarial wording and return instructions. If several witnesses have separate affidavits, list each signer rather than assume one person can sign for the group.

What is the difference between a sworn statement and an acknowledgment?

A sworn statement involves an oath or affirmation about the statement. An acknowledgment is a different notarial act concerning the signer’s acknowledgment of execution. Your document preparer or recipient should identify the required act and certificate; ask before signing in advance.

Can the finished document be returned to our office?

Discuss printing, scanbacks and original return when requesting the appointment. Include the recipient and deadline so the quote reflects the full handling plan. The notary does not evaluate the underlying claim or negotiate its terms.

Prepared affidavits for claims and business matters

For an insurance affidavit, proof-of-loss statement or business declaration, obtain the document and instructions from the requesting organization or attorney. Tell us whether an oath or affirmation is requested and whether the signer must sign at the appointment. We review the notarial act without deciding the claim or confirming the truth of the statement.

Arrange a Boston-area mobile visit

Request a home or office appointment in Boston, Cambridge, Somerville, Brookline or another agreed Greater Boston location. Include the signer count, complete packet, deadline and any requested original-document return. If an insurer or law office needs several affidavits from different people, identify each signer so timing and travel can be quoted accurately.
